What do I need to bring to my consultation appointment?
●     A Valid Colorado Photo ID or Driver’s License
●     Medical Documentation supporting your condition (CLICK HERE to find out how to obtain your medical records)
●     Personal check, cashier’s check, or money order made out to CDPHE for $90.00
●     Doctor recommendation visit $110.00 cash or credit card (credit card fee applied)
